Appointments

To make an appointment, please call the clinic where you would like to be seen. If your insurance plan requires, you may need a referral from your primary care physician or the physician currently treating you before receiving an authorization from your insurance company to make your first appointment.

Your First Appointment

 We recommend that you arrive 15 minutes prior to your appointment time. If you are unable to make your appointment, please contact our office as soon as possible.  Please bring copies of all of your insurance cards to all your appointments.  Co-pays will be collected for all visits as appropriate.

 

A number of forms are required to be completed at your first appointment and at subsequent appointments.  For your convenience, you may complete several of them prior to your appointment.  Note that other forms are duplicate/ triplicate and may only be completed in person at the time of your appointment.

Physician Contact Form for Neuro-oncology (PDF) – please download and complete with information about doctors treating you and bring it with you to your appointment. 

 

Medication and Allergy List (PDF) – please bring a list of all current medications, noting their dosages/ strength and frequency. Since this is a form requiring your original signature  for your medical record, you will be asked to copy this list onto the UCSF Medication form at the time of your visit.
